Hopewell House is an independent, nonprofit residential care home serving individuals receiving hospice services. We work in close partnership with hospice providers and care teams to offer a supportive, home-like environment with 24/7 caregiving support for residents and their families.
Hopewell House is not a hospice provider—we are a home.
As a Residential Care Facility (RCF), we provide round-the-clock care through
a dedicated team of professionals and volunteers. We collaborate with community hospice providers, who oversee each resident’s medical hospice plan of care while visiting regularly.


Our care model is built on close collaboration. Hospice providers work directly with our care team to coordinate each resident’s care plan. Outside partners enhance our team and resident experience. Together, we create a compassionate and connected end-of-life experience.
All residents must be receiving hospice services and are typically in the final phase of life—whether that be months, weeks, or days. Length of stay is not restricted, as long as hospice services remain in place.
Hopewell House is not a hospital or memory care facility. Each referral is carefully evaluated to ensure we can safely and appropriately meet the resident’s needs.


Residents must be enrolled with a hospice provider prior to admission. In many cases, hospice enrollment and admission occur on the same day. When possible, we encourage early hospice enrollment once it is recommended.
Because needs can change quickly, room availability may shift. Providers are encouraged to contact us directly to discuss current openings and referrals.
